Add/Remove items from the table of contents list manually.

For example, we could click the icon next to a block and choose "add to table of contents" or "remove from table of contents".

Currently, because Headings are the only way to get larger text, if you want larger text for visual purposes, it gets added to the Sticky Table of Contents, even if you don't want it there.
