New task modal, custom field menu needs redesign

In the "new task" modal, when you click "custom fields" it shows this interface. At the moment it's a bit messy, it probably needs a redesign.
Having to click to show empty custom fields is counterintuitive as they're obviously empty if the task is new. Also the "add or edit fields" button at the moment it's out of place, bigger than needed and probably unnecessary.

Also, very important, from this model I'm not able to access relationship custom fields. This option should be added.
