Hey all, just wanted to give you all a transparent update on the status of this request. We are currently in the SCOPING Process, and are taking all of your survey results into consideration. Thank you to each and every single one of you who has added your thoughts and opinions! We will directly use this feedback to build out the feature the exact way that you all have a use for it!

Believe me when I say this, I hear the feedback each and every single one of you has provided to our team! I agree with each and every single one of you fully, this will be a huge unlock, for service teams and businesses on ClickUp alike!

Problem:
Currently, ClickUp Forms do not support Relationship Custom Fields, which prevents users from linking form submissions to related tasks or lists during the intake process. This limitation forces users to either:
Use workaround dropdown fields that don't provide rollup data
Manually add relationships after form submission
Use external form tools with integration middleware
Use Case:
Many teams use ClickUp as a relational database where one list contains records (clients, vendors, projects, locations, etc.) that need to be referenced by tasks in other lists. When collecting information through forms, users should be able to select from existing records in related lists. For example, linking a new request to an existing client record, or associating an event with a specific location from a master database. Without relationship field support in forms, the rollup data from those related records isn't available on the newly created tasks.
Requested Feature:
Enable Relationship Custom Fields (both "Any task in your Workspace" and "Tasks from a specific List" types) to be added as form questions. When a user fills out the form, they would see a searchable dropdown of available tasks from the related list, similar to how relationship fields work within tasks.
Benefits:
Streamlines data collection by capturing relationships at intake
Reduces manual work and human error
Enables rollup data to be available immediately on form-generated tasks
Creates more powerful database-like functionality within ClickUp
Maintains data integrity by linking to existing records rather than duplicating information
Current Workaround:
Using standard dropdown fields with manual relationship linking after submission, which defeats the purpose of automation and creates extra administrative overhead.

This is the main functionality I am choosing between Monday and Clickup. Well put Bryce. I am so confused this is not possible at the moment.

If a form has an 'email' field it would be great to then map this form to the person with the same email in the CRM, this would connect things all together (the person in the CRM, their form submissions, support tickets, projects ect..)
