Our team is looking to create ATPs to test several projects across the company's portfolio. The team is looking to leverage forms to do so with a custom field called "Pass/Fail". Due to the current limitations of Clickup forms, the team would need to create several custom fields that are all used once per item of the ATP. It would be ideal if custom fields could either be form specific or have an option that allows them to be used multiple times within a form.

Need : Configuring templates so that fields generated from a form appear in specific areas throughout the description of the task (or doc) automatically generated from submitting the form. Example of use : Get a full copy of the contract/consent ClickUp form signed by the client (with form text and client's answers at the right place within the text). Currently used workaround : Check the option to Add custom fields to task description, combined with a template including a copy of the form's text in the description, then manually copy/paste each custom field where it shoud appear in the form's text. This takes way to long. Workaround suggested by ClickUp's Bot : Use Zapier. (Yet to try out) It would be easier to simply have the option to set custom fields text to be added throughout a doc and description text, and also have the option to print to PDF the entire form response as is with the form text. I spent hours setting up the logic for one of our questions. I wanted to then duplicate what I had done to use three more times in the form. I found out, I have to re-create the 15 rules of logic over and over for each of the same/similar questions. For example: The form is for customers wanting to start a project. We ask them what product they want to create first and have logic pull different questions for each product. Often, for one project, the customer may want to do two to four products under one project. This is where being able to duplicate a question with all the logic included is invaluable.

On the ClickUp form, we can add a question description to fields, as shown in the screenshot, which allows us to provide a clearer explanation for certain questions that we ask. Currently, this is not available for fields that show up based on certain conditions. This would be extremely helpful to add a question description based on the condition.

While this makes it easy to collect responses, it may not meet the requirements of some teams or industries.
